In the ICD-10 book under personal history of malignant neoplasm the instructions state to code first any follow up examination after treatment of malignant neoplasm (Z08), however, both the personal history of malignant neoplasm as well as the follow up code have an edit that shows they unacceptable as the principal diagnosis. What should be used as the principal diagnosis if all you have is personal history of malignant neoplasm?
